(we may not be able to honor all requests).

E. Accounting of Disclosures

Request a list of times we shared your PHI.

F. Receive a Paper or Digital Copy of This Notice

G. File a Complaint

If you believe your privacy rights were violated, you may file a complaint with:

  • EMRA Aesthetics

  • The U.S. Department of Health and Human Services (HHS)

You will not be penalized for filing a complaint.

5. How We Safeguard Your Information

We use:

  • Password-protected systems

  • Secure encrypted platforms (such as GoHighLevel)

  • HIPAA-compliant storage

  • Staff training on privacy

6. Changes to This Notice
